LT1045 Deck Pulley, PTO or Other Problem

I have a 2007 (?) LT1045 (Serial # 13AX11CH712) Belt # 954-04033(A). This mower was given to me used about 3 years ago. Its an excellent mower, and has been great to me in the past.
At the end of the 2018 cutting season the belt would start to come off intermittently while I was cutting the yard, and I usually cut my yard on the number 4 setting. I bought a new belt at the beginning of the 2019 cutting season. After the new belt, about the second time cutting it would come off, even at the deck set to the highest setting. At that point I replaced the tension bracket, spring and bolt hardware. It worked for about 2 more cuts and now it wont stay on at all. I have leveled the deck (using the manual as a guide) and still no better. I really don't want to spend a bunch of $$ "trying" to fix the issue, and end up I could have purchased a new mower. I am really struggling with this and I don't like to be beaten by something.
