9/16-18 is just the threaded portion of the axle bolt. It has a very long shoulder on the bolt (I don't remember what the shaft diameter or length of the shoulder is off the top of my head). If you look at the axle bolt holes on the frame, one is large and the other is small. The smaller bolt hole is for the 9/16 end.
I kinda buggered up the threads on my cco axle bolt when I took it out. I looked around for a new one, but ended up repairing the threads because it was easier.
For a CCO axle bolt, I vaguely remember they could be purchased for new for $25 somewhere online, but can't find it now. They pop up on ebay every now and then.
You could possibly put together a bolt and a sleeve/guide to make it work. I tried to, but couldn't find the right sizes at my hardware stores. You'll have to measure the inside diameter of the pivot point on the axle. I can't remember if it was 1" or 1 1/8" or somewhere in there.
Edit: Anyone else have some measurements or ideas? I just posted what I remember about it. I won't have access to my original to check for measurements on the bolt for a few weeks.
