Fabrication of rear lift attaching plate (IH-384679-R11 )

Has anybody fabricated one of these? It seems easy enough but I don’t have an idea of the dimensions. If anybody could measure theirs or knows of some plans somewhere I’d greatly appreciate it. I need one today and I thought I had it but turns up I don’t.
